During the second stage Mesothelioma, fluid starts to build up between the membrane of the lungs and the membrane of the chest wall, resulting to pleural effusion. The increase in the volume of fluid produced causes shortness of breath and chest pain. Other Mesothelioma cancer patients experience dry and persistent cough. Diagnosis of the pleural effusion is achieved through a chest x-ray.&lt;br /&gt;&lt;br /&gt;Stage III Mesothelioma means that the malignant cells have already spread to the chest wall, esophagus and the lymph nodes on one part of the chest. Fluid in excess of that required for lubrication is removed through the blood &amp; lymph system. In a person with Mesothelioma, the asbestos fibres penetrate the organ, into the fluid sac, producing cancerous cells which attack other cells, thickening the fluid, causing pain, and difficulty with breathing. Peritoneal Mesothelioma and, less commonly, Pericardial Mesothelioma, are not as common as the pleural type, but just as deadly.&lt;br /&gt;&lt;br /&gt;The Causes of Pleural Mesothelioma&lt;br /&gt;Pleural Mesothelioma, the most common form of mesiothelioma, is caused, according to all known evidence, exclusively by inhalation of asbestos fibres. These fibres are very thin &amp; sharp enough to penetrate the lining of the lung, entering the plural sac, thereby damaging the mesothelium cells. Often, cancerous growth results, as described above. Other factors also come into play, including the changes induced in macrophages by their ingestion of asbestos particles. The asbestos seems to stimulate the macrophage to produce free radicals, which then affect DNA to induce cancerous cell behaviour. The chances of a person in an asbestos-related environment developing lung cancer, are said to be about 50 times increased if that person smokes, though there is no evidence to show that smoking itself causes mesothelioma. Ironically, one brand of cigarettes in the 1950's used asbestos in the filter tips.&lt;br /&gt;&lt;br /&gt;There are other factors too that can play a part, such as heredity, general health and diet. Although not too much has been clincially proven about these factors, as with all diseases, the better the person's health the better their chances of not catching a disease.&lt;br /&gt;&lt;br /&gt;The History of Asbestos. It is probably not generally known that asbestos has been around since 4000 BC, when it was used in lamp wicks and candles. Interestingly, it was also used in the cloth wrapping of Egyptian mummies. Maybe this is the real origin of 'The Curse of The Mummy' stories; grave robbers, and maybe Egyptologists, could expect to get their just desserts! Clothing made from asbestos was highly prized in ancient times to wrap the bodies of Kings, so that their ashes would not mix with the earth or other contaminants during cremation, and it is said the Romans simply threw their asbestos napkins into a fire to cleanse and purify them.&lt;br /&gt;&lt;br /&gt;Asbestos, a type of silica, is mined from three main types of metamorphic rock:- Chrysotile (white asbestos), Amosite (brown asbestos) and Crocodilite (blue asbestos). Other asbestos bearing rocks such as Tremolite, Anthophyllite and Actinolite are not in such common use but could still be found in some construction work, talcum powders and vermiculite. During the Middle Ages there does not appear to be much evidence of asbestos use; they probably had other problems, like recurring plagues and crusades, to contend with.&lt;br /&gt;&lt;br /&gt;The fact that asbestos workers developed respiratory illnesses, and died young, was known as far back as Roman times when Pliny the Elder commented that asbestos workers seemed to have many health problems, advising against buying asbestos-workings slaves as they "died young". The deleterious effects of asbestos, in modern industrial times, have been recorded as far back as the start of the 18th Century but little notice was paid by factory owners &amp; Governments to the plight of asbestos workers - labour was cheap and life even cheaper; nothing should stand in the way of the sacred cows of Industrial Progress and the profits of industrialists and politicians.&lt;br /&gt;&lt;br /&gt;That it took until the 1970s before more stricter legislation to restrict its use and safeguarded workers was drawn up, is scandalous. A charitable reason for this inertia may be that asbestos had become so widely used in almost every aspect of our everyday lives, delivering such tremendous benefits, that there was such a reluctance to finally admit that something so useful could also be so deadly. People who have expounded the virtues, and made a lot of money out of, asbestos are hardly likely to suddenly throw up their hands and admit to being wrong. The alternative, less charitable, explanation can only be that greed and profit were more important than employees' health.&lt;br /&gt;Deja vu!&lt;br /&gt;&lt;br /&gt;There are many ex-asbestos industry workers who are living time bombs, not knowing if the deadly fibres have affected their health. I had the excitement of youth and looked on the thing as a big adventure.&lt;br /&gt;&lt;br /&gt;The mine was located about 7 miles from the Alaska border on the banks of the Forty Mile River, which emptied into the Yukon River some 5 miles or so north.&lt;br /&gt;&lt;br /&gt;I first arrived in August and started working on the Surface Crew. This was the group responsible for outside maintenance of all things on the surface. This was fine when we had those few relatively warm days in summer, but later it was a bit more difficult to be working outside in minus 55F.&lt;br /&gt;&lt;br /&gt;One of my first duties was to keep clear the conveyor that took the unused asbestos out to the tailings piles. Tailings piles are basically the dregs of what's left over after milling the asbestos and is run out of the mill on conveyor belts to large piles behind the mill. At that time they were about 100 feet high.&lt;br /&gt;&lt;br /&gt;I was given a shovel, and a small mask (kind of like the little paper ones that painters use) and told to go shovel off the conveyor belt on the tailings piles. Dutifully I climbed up with my little shovel to clear any blockages of asbestos from the conveyor. I remember clearly at one point standing literally knee deep in asbestos on top of this 100 foot high pile and looking inside my very poor quality mask and seeing the inside (where it's not supposed to be) all grey from the asbestos dust. I then took out my hankerchief (yes, I carried one) and blew my nose. Sorry for the rude, graphic description, but it was all grey. And that was my introduction to work at an asbestos mine.&lt;br /&gt;&lt;br /&gt;Even back in the early 1970's it was becoming known that asbestos caused problems. Working in the mine we'd get brochures handed around periodically with propoganda about how it was never proven that asbestos was actually harmful. They were beautiful glossy brochures. I wish I'd kept one.&lt;br /&gt;&lt;br /&gt;Though I had occasion to go into the mill for various reasons I was glad I didn't work there. There were employees that worked in the mill whose sole job was to sweep up the dust that fell on the floor. There was so much of it that this was a constant ongoing job. The asbestos dust in the mill actually fell almost like snow and covered the floor completely. Without sweepers there would probably have been several inches of asbestos dust on the floor within an hour or so. In fact, I remember seeing sweepers go by pushing their wide brooms and the new dust settling onto the floor behind them as they walked.&lt;br /&gt;&lt;br /&gt;The Yukon itself was absolutely beautiful. Stunning in fact. I had many great experiences there and saw some natural wonders I couldn't have seen anywhere else. I had hitchiked up the Alaska Highway with a friend. In those days the highway was unpaved. Most of the trip was provided by a nice family who were travelling in an old converted school bus. Rattle and dust. Rattle and dust. But we made it finally to Whitehorse, and then on to Dawson City. I loved Dawson. It was like stepping into the past. Though they may not show these reactions outwardly, they almost certainly will be thinking about them. Don't assume that just because they appear 'brave' on the outside that they are 'taking it well'.&lt;br /&gt;&lt;br /&gt;Some patients will find it easier to talk with family and friends about their future than others do. Doing this may help the patient themselves come to terms with the diagnosis. Conversely, there are those that find it difficult to talk about it to anyone. It that is the case then let them be. The patient has the right to be firm on the issue, in the short term anyway.&lt;br /&gt;&lt;br /&gt;On the other side of the coin, friends and relatives may appear to be denying the diagnosis, changing the subject and playing down the patient's worries and anxiety during conversations. The disease is difficult to treat and in most cases the prognosis is poor. Treatment options include:&lt;br /&gt;&lt;br /&gt;1. If the diagnosis is done in the early stages surgery may be recommended to remove all cancerous tissues. This means thoracoscopy, VATS or video assisted thoracic surgery, mediastinoscopy (used for staging), or laproscopy. Often, doctors will advise palliative procedures like chest tube drainage and pleurodesis, thoracoscopy and pleurodesis, pleuroperitonial shunt, or pleurectomy, which treat the symptoms of mesothelioma rather than the disease.&lt;br /&gt;&lt;br /&gt;2. Radiation is prescribed aggressively for mesothelioma patients and is often given in combination with surgery or in order to control symptoms, palliatively. Research on using radiation therapy using implants or UV light therapy is in progress as traditional radiation therapy damages surrounding healthy tissue.&lt;br /&gt;&lt;br /&gt;3. Chemotherapy is an option and around 12-20% of patients respond to the drugs. Anti-cancer drugs destroy cancer cells and prevent their spread. In mesothelioma, chemotherapy is not considered to be curative. The aim is to prevent spread of the disease; shrink the tumor prior to surgery, known as neoadjuvant therapy; to annihilate any remains of the tumor in the body post surgery; and to relieve pain and other discomfort, palliative chemotherapy. Experts recommend prescribing premetrexed along with cisplastin. These drugs have shown positive results and this is now standard care for Mesothelioma not treatable by surgery.&lt;br /&gt;&lt;br /&gt;4. Biological therapy using interleukin 2.&lt;br /&gt;&lt;br /&gt;5. Immunotherapy where the body’s own immune system is stimulated to fight the disease. Biological response modifiers, BRMs, are used in treatment. These enhance the functions of the immune system; regulate metabolic reactions that promote growth of cancers; alter cancer cell division; block or reverse processes that lead to formation of cancers; and prevent spread of cancer.&lt;br /&gt;&lt;br /&gt;6. Gene therapy, where attempts are made to correct the gene sequence that causes the cancer. This is of two kinds, replacement gene therapy and knockout gene therapy.&lt;br /&gt;&lt;br /&gt;7. Administration of lovastatin, a cholesterol lowering drug.&lt;br /&gt;&lt;br /&gt;8. Photodynamic treatment where a photosensitive medication is activated using a laser. This is done during the surgical removal of cancerous tissues.&lt;br /&gt;&lt;br /&gt;9. Draining of fluid in the chest or abdominal cavity followed by introduction of medications into the cavity to prevent fluid accumulation is done for patients who have uncontrolled fluid accumulation and intense discomfort stemming from it.&lt;br /&gt;&lt;br /&gt;10. Combination therapy or Trimodal therapy which means surgery accompanied by radiation and chemotherapy.&lt;br /&gt;&lt;br /&gt;Often, complementary medicines are also used. This is followed by Intensity modulated radiotherapy (IMRT).&lt;br /&gt;&lt;br /&gt;What is really unique about this new mesothelioma treatment study is the fact that this is the first trial to investigate the use of chemotherapy before surgery and radiation.&lt;br /&gt;&lt;br /&gt;Results Of Previous Trials Of This New Mesothelioma Treatment&lt;br /&gt;&lt;br /&gt;Previous trials of this new mesothelioma treatment involving extrapleural pneumonectomy and IMRT was 90 per cent effective in preventing a local recurrence in patients. However about half the patients who underwent the trials got a recurrence in a different location.
